A diet low in FODMAP’s may be of benefit to some IBS (Irritable Bowel Syndrome) sufferers as it removes or alleviates symptoms of bloating, wind, abdominal pain with alternating bowel habits between constipation and diarrhoea with or without nausea and vomiting, but not all! Neither will it remove other physiological symptoms such as skin conditions, joint pain and discomfort, headaches or migraines.

A diet high, FODMAP’s (fruits, dairy, vegetables and grain cereals) are a category of carbohydrates, which rapidly fermented in the gut/ intestine.

It is important when speaking with your GP that you ask to be referred to a registered dietician with FODMAP approach training.

The low FODMAP diet is complex and so it is advisable to speak with a dietician, and one who is registered and experienced with this specialist diet. This is important as it is all to easy to begin a diet but omit certain foods which subsequently will also mean that either we do not benefit fully or we miss out on essential vitamins and minerals, this can have a detrimental affect on our body through lack of nutrition. Without the appropriate knowledge of foods, ingredients, processed foods, labels and eating out we can easily read information wrongly as some ingredients are ‘hidden’ being called different names like E numbers. There is a lot of confusing advice, some research has become outdated, then there is new research, and it is all too easy to read information on the internet to follow it to our detriment or even peril. It can also become ineffective.

The FODMAP diet is relatively new, so it is possible that a GP or gastroenterologist does not know if this diet, ask for them to refer you to a registered dietician to receive the correct up-to-date information. There are FODMAP trained dieticians working in the NHS and privately both may require referrals from your GP or consultant private dieticians may also require a referral. A vast amount of research and training is being undertaken a growing number of registered dieticians who are skilled in the delivery of the low FODMAP diet with formal training at a variety of global institutions.

What is a food allergy or intolerance?

A food allergy is sometimes used to describe all adverse reactions to food, the term is more often used to refer specifically to food reactions that are mediated by the immune system.

To protect us from illness and disease, our immune systems protect us from illnesses and diseases so  are continuously trying to lessen the danger represented by substances called antigens. Antigens are parts of proteins that our bodies recognize as dangerous and take steps to neutralize. Antigens can be found most anywhere there is protein – in foods, of course, but also in microorganisms like bacteria.

When our immune cells identify a dangerous antigen, they act to neutralize it and prevent it from causing harm in the body. When antigens from bacteria or viruses interact with our cells, we can get the flu, or the common cold. We don’t get the flu from food antigens, but we can get a wide range of immune-related symptoms that range from sniffles to hives to anaphylactic shock.

Immediate versus Delayed Hypersensitivity

Allergic reactions to food, also called food hypersensitivities, are further classified as either immediate or delayed. Immediate hypersensitivity reactions occur within hours or even a few minutes after a food is eaten, typically causing very obvious physical symptoms such as a rash, the hives, a running nose, or a headache.

In rare cases, immediate hypersensitivity reactions can cause anaphylactic shock, a life-threatening condition in which the throat swells and blocks the passage of air. Immediate hypersensitivities affect only a small percentage of the population.

Immediate Reactions to Food

The foods that are most often implicated as the cause of immediate allergic responses include milk, eggs, peanuts, tree nuts (walnuts), soy, strawberries, wheat, fish and shellfish. Many people with immediate food hypersensitivities must completely eliminate the offending food from their diet to avoid the serious symptoms.

Delayed Reactions to Food

Many of the same foods that are known to cause immediate hypersensitivities in a small number of people, have been implicated as a cause of delayed or “masked” food allergies in much larger numbers of individuals. Delayed food hypersensitivity reactions are believed to affect millions of people; some physicians have suggested that as many as 60% of all Americans suffer from masked food allergies.

These reactions may be responsible for a variety of symptoms including dark circles or puffiness under the eyes, fluid retention, dermatitis, sinus congestion, fatigue, abdominal pain or discomfort, joint inflammation, mood swings, indigestion, headaches, chronic ear infections, asthma, poor memory, anxiety and depression.

As the name suggests, delayed hypersensitivities do not appear immediately after consuming a particular food. In fact, in most cases the immune response is so delayed that it is difficult to determine which food is causing the symptoms, and many people are unaware that they are sensitive to certain foods.

Only through careful dietary manipulation, such as an Elimination Diet or Rotation Diet, is it usually possible to identify these hidden food allergies. The foods most often associated with delayed hypersensitivities include dairy products, eggs, wheat, soy products, peanuts, shellfish, and refined sugar.

Food Intolerance are immune-mediated food allergies represent one type of adverse food reaction. Another type of adverse food reaction is called food intolerance. Food intolerance is an umbrella term that refers to any abnormal physiological response to a food that is not caused by an antibody/antigen reaction. For example, some food intolerances are caused by enzyme deficiencies, while others are caused by poor function of the digestive tract or a sensitivity to a natural or synthetic chemical.

Lactose Intolerance is the most common food intolerance, which affects as many as 30% of adults, additionally it is particularly common in people of African and Asian heritage.

People with lactose intolerance do not produce enough of the digestive enzyme called lactase, which breaks down the milk sugar (lactose) found in dairy products. When too much undigested lactose makes its way into the large intestine, people suffer from gas and/or diarrhoea.

Wheat intolerance,  wheat allergy, and wheat sensitivity are all terms frequently used to described adverse reaction to this food. Wheat is unique when it comes to adverse food reactions, particularly because it has long been classified as the primary “gluten grain” and because its research history has been both complicated and controversial. Understanding allergy-related issues associated with gluten is important for understanding problems connected to wheat.

Coeliac disease symptoms vary among people as a result the symptoms can range from very mild to severe. The reactions are different from an allergic reaction as symptoms do not cause anaphylactic shock. Coeliac disease is a ‘multi-system’ disorder for the reason that symptoms may affect any area of the body.

Symptoms

Symptoms of eating gluten differ among individuals which can last from a few hours to a few days whilst eating gluten so less gluten makes your symptoms vary in severity:

• Headaches,
• Diarrhoea,
• Stomach pains, cramps, bloatedness,
• Lethargy,
• Severe or occasional diarrhoea, excessive wind and/or constipation,
• Persistent or unexplained gastrointestinal symptoms, such as nausea and vomiting,
• Recurrent stomach pain, cramping or bloating,
• Deficiency of one or a combination of iron, vitamin B12 or folic acid,
• Anaemia,
• Tiredness and/or headaches,
• Unexpected or rapid weight loss,
• Mouth ulcers,
• Alopecia (hair loss),
• Dermatitis herpetiformis (skin rash)
• Tooth enamel problems,
• Depression,
• Infertility,
• Liver abnormalities,
• Repeated miscarriages,
• Joint pain and/or bone pain,
• Neurological; nerve; ataxia which is poor muscle coordination; neuropathy numbness and tingling in the hands and feet,
• Amenorrhea which is a lack of periods in women,
• Symptoms in young children and babies may differ and require close monitoring to refer to a GP.

Chronic diarrhoea ~ signs, symptoms, diagnosis, treatment


Diarrhoea is labelled chronic by a doctor, when it occurs at least three or more times daily for more than four weeks! That’s a long time to suffer believe me!
What are the causes chronic diarrhoea?
Chronic diarrhoea can often be a result of or caused by a symptom of an illness, infection, or another condition; known or unknown to the sufferer. Increasingly the number of people who have a reaction to food having difficulty or being unable to digest particular foods: lactose the sugar found in dairy products, lactase the protein found in dairy products, fats, caffeine, alcohol, acid, yeast, chemicals and much more. Another cause may be from infections caused by bacteria, viruses, or parasites found in food or travelling abroad where we are not used to theses living organisms. I have found medicines such as laxatives and antibiotics cause chronic diarrhoea and apparently; cancer treatments, heart medicines, minerals; magnesium, can cause chronic diarrhoea in some people. In addition to this, medical conditions that affect the stomach or intestines: inflammatory bowel disease; IBS; Thyroid, pancreatic or liver disorders; cancers can also cause chronic diarrhoea; autoimmune diseases for example, coeliac or crohn’s disease, often cause periods of chronic diarrhoea. Having any surgery or procedures whether medical or cosmetic: including any invasive stomach or bowel surgery, especially where damage to muscles which control bowel movements is caused. I believe having Caesarean sections cause this, but the doctors and consultants cannot comment until more research is done to confirm. Which reminds me that, Medicine is a relatively new Science

The signs and symptoms of chronic diarrhoea are severe abdominal pain with an overwhelmingly urgent need to have a bowel movement or even worse a loss of bowel control. Additionally, there may be a small amount of blood, mucus, or pus in your bowel movement which you need to notify your doctor about when considering the cause of the chronic diarrhoea. Due to going to the toilet three or more times a day for four weeks it is inevitable that some weight loss will occur (this varies in sufferers). Also due to the increase in frequent rapid bowel movements then some anal discomfort is likely to be caused by sores with irritation and inflammation – zinc and castor oil cream or sudocrem will alleviate this somewhat or visit a pharmacist who will advise.

Treatment depends on what the condition is which is causing your chronic diarrhoea.

Treatments:

Oral rehydration therapy
You must drink more liquids to replace body fluids lost through diarrhoea to prevent dehydration.
You may be advised of your need to drink an Oral Rehydration Solution, (ORS) especially babies, young children, pregnant women or the elderly. As the Oral Rehydration Solution has the exact amounts of sugar, salt, and minerals in water which are needed to replace body fluids.

Anti-diarrhoeal medicine
This is given as it decreases the frequency and amount of diarrhoea you are having by coating the intestine (bowel) so making the stool less watery or working by slowing down how fast the intestine is moving.

Discuss with your practitioner if one medicine does not work you may require one which slows down the bowel movements, peppermint tea also aids slowing bowel movement which I prefer over drugs.


Chronic diarrhoea is diagnosed by a general practitioner (doctor) or nurse initially they will examine you, ask about your symptoms; make sure you say if have noticed any: unusual colour or smell to your bowel movements; blood, mucus, or oil droplets in your bowel movement, after eating certain foods; if you have travelled abroad recently; if who you live with or are close to has had the same symptoms.
It is therefore routine to take tests to eliminate or show certain causes of your symptoms:

Blood sample is sent to a laboratory testing that major internal organs, e.g. kidneys, are working correctly.

Sample of your bowel movement is sent to a laboratory to aid finding the possible cause of your chronic diarrhoea.

X-ray of your abdomen to show if there is a possible cause of your chronic diarrhoea.

CT scan or CAT scan, this is an x-ray machine which uses a computer to take photographs of your abdomen, which may show the cause of your chronic diarrhoea. A dye is given before the pictures are taken to help the practitioner to see the photograph better.

N.B. The caregiver needs to know if you have ever had an allergic reaction to contrast dye.

Colonoscopy or sigmoidoscopy is a procedure to examine the inside of the colon which is the large intestine). A thin flexible tube with a small light and camera on the end is used. The procedure is normally shown on a screen for the doctor which you can watch if you wish. A biopsy and photographs may be taken to eliminate possible causes and for future reference.

Look after your insides ~ stomach, intestines…

1. Eat healthily.

2. Eat regularly.

3. Eat sitting up at a table, preferably.

All too easily we spend our daily and/or working lives gulping down food between events/lessons/meetings/exercise. In addition, after returning home we then spend the evenings on our electronic devices and or/sitting in front of the TV with a takeaway or additional snacks in the evening. The problem is that by eating this way we can cause problems with our digestive system, as food does not digest properly so some people have lower stomach and abdominal cramps or indigestion (which if severe is akin to a heart attack).

4. Stop smoking.
Smoking can weaken the muscle that controls the lower end of the oesophagus (food pipe or gullet), causing heartburn.

5. Lose excess weight.
If you are overweight, your tummy fat puts pressure on your stomach this can cause heartburn.

6. Exercise regularly.

6. Do not binge drink.
Binge drinking increases acid production in your stomach which can cause heartburn, as well as making other digestive disorders worse.

7. Beat stress.
Anxiety and worry can upset the delicate balance of digestion exacerbating so worsening digestive conditions such as irritable bowel syndrome.

Diarrhoea sufferers know the disruptive embarrassing uncontrollable emergency which can be a nightmare to cope with I seek to help to assist to eliminate this.

I do not know about you but often health professionals do not understand until they or someone close to them suffers – often their mother and then they DO something about it!

What is diarrhoea?

Signs may include some of the following:

  • Abdominal pain or cramps or a bloated stomach.
  • Passing of loose, unformed or liquid stools when you go to the toilet
  • Sudden urgent need to rush to the toilet several times during the course of a day
  • Nausea or feeling faint
  • vomiting
  • high temperature of above 38ºC (100.4ºF)
  • headache
  • loss of appetite

Symptoms of dehydration in adults:

  • lacking energy or feeling tired
  • loss of appetite
  • nausea
  • feeling light-headed
  • dizziness, especially when standing up
  • dry tongue
  • sunken eyes
  • muscle cramps
  • rapid heartbeat

How can you differentiate between acute and chronic diarrhoea?

ACUTE diarrhoea:

  • Starts with little or no warning
  • Clear up of its own accord in 1 to 3 days
  • There may by cramping pains, bloating or wind

Sufferers rarely contact of General Practitioner as it passes with cutting out dairy, food; eating a BRAT diet of Bananas, Rice, Apples and Toast or the use of over the counter drugs such as Imodium; if it is not a virus or food poisoning which has caused it.

CHRONIC diarrhoea:

  • Is ongoing, persistent, prolonged or recurrent symptoms with cramp, bloating, pain
  • It may occur infrequently or more frequently despite use of Anti-diarrhoeal and Antispasmodic drugs such as Imodium which have no difference to the symptoms as a maximum amount can be taken and more than this and the drug will not work any more than previously
  • Requires medical advice  due to lose of fluids and vital minerals and nutrients (especially dangerous to the young and elderly)
  • Debilitating
  • Possible signal of  the presence of a potentially serious internal digestive health problem

Food enters the mouth being broken down by teeth and enzymes in saliva which then goes down the food pipe entering the stomach leaving as a kind of lumpy porridge consistency into the small intestine where nutrients are absorbed into the blood for the body then the remaining undigested fibre enters the large intestine through the walls of this nutrients and water are absorbed  but if this is too fast then water and nutrients are not correctly absorbed as such making a loose watery bowel movement  when the stool released. The internal intestines continue to break down the food but something upsets the process so the intestinal muscles speed up the digestive process causing diarrhoea Imodium slows this down as does Peppermint tea which enables the intestinal muscles to contract or the sufferer will become ill from dehydration if the lost fluids are not replaced quickly.

There are inconclusive opposing views of the treatment by doctors and medical staff. One view by my local doctor would now say let the diarrhoea continue in order to flush out your system getting rid off the bad bacteria or whatever has caused the bout; ensuring that you drink plenty of fluids and Diaorlyte if required or ‘flat 7UP’ works just as well! This works for bouts caused by viral or food poisoning which need to be cleared out of your system. I have treated my children and myself with 7Up, camomile tea, peppermint tea, Cardamon pods boiled in a little Marigold vegetable stock and added with a little pasta shapes when recovering to give energy. While the opposing view is that diarrhoea is not your body’s natural way of cleansing or getting rid of bacteria or other ‘nasty things’ and therefore you must resolve the problem to ensure a speedier recovery rather than letting it take its natural course – this view promotes use of drugs to alleviate and doctors would use this to treat chronic diarrhoea as opposed to acute cases. In this case I have used Imodium however after 8 tablets it will not work, eliminate foods which have caused my bout; gluten, dairy, fruit, vegetables.

It takes a month to renew the lining of your intestines. Therefore if food has caused it, I would be very careful to not eat anything which may cause further problems. As the digestive system is highly tuned so a tiny change in the speed of the intestines process food can lead to an attack of diarrhoea as a result I have found that by trying to understand what is occurring in my body,  then I can prevent or control a recurrent bout. However, when you suffer from chronic diarrhoea as I have you begin to decide that you are better removing what is causing the problem rather than continually taking drugs and having your life disrupted by it preventing holidays, events, outings, travelling when it is caused by food intolerance or allergy, a virus or bacteria; excess food, excess alcohol, a hormonal imbalance or a surge of adrenaline due to stress as I will go into more detail in the next paragraph.

~ When does it happens?~

These are a few of the cause, however it is important to note that everyone is different as some people will suffer while others do not appear to as their system has the opposite effect being constipated which is another topic altogether!

A common cause and the first to rule out often is Stomach bug/food poisoning which can be caused by a number of different viruses and bacteria infecting your bowel and give you diarrhoea; usually due to food that’s either contaminated, gone off, or not cooked properly such as rice, fish, chicken, cheese, dairy, kebabs, burgers etc. Rarer and more serious cases are caused by cholera or dysentery usually abroad.

It can be due to nerves and anxiety before a test, exam interview, wedding, travelling, extended periods of stress and tiredness causing mental stress. due to the adrenaline  pumping around your body speeding everything up which upsets the rate and therefore balance of their digestive system leading to diarrhoea, the rate food passes through the digestive system; referred to as Performance Anxiety Diarrhoea (PAD).

Food intolerances commonly cause a bout of diarrhoea to the sufferer which can be triggered by a sensitivity to certain foods which you may have as a baby, child or develop later in life. The three most common food intolerances are:

  • Gluten intolerance  which is a sensitivity to a substance in wheat, barley and oats
  • Lactose intolerance when you don’t have enough of the enzyme ‘lactase’ which digests milk sugar
  • Allergic reactions especially to foods such as shellfish, soya, nuts, eggs, celery, acids

An over indulgence of eating and drink to excess especially at Christmas for some can easily irritate the lining of the gut; rich or spicy foods, too much alcohol, excess fibre and large amounts of acidic fruit or fruit drinks are the most frequent causes.

For women around or during menstruation seems to be a common experience diarrhoea once a month usually around the time your period or either side the reason being a hormonal imbalance resulting in your gut more sensitive than normal.

Irritable Bowel Syndrome (IBS)   sufferers experience diarrhoea as one of the symptoms and many will suffer at one time or other in their life sadly the doctors have no known cure or reason as a result it is extremely irritating both suffering and seeking to eliminate causes as the other symptoms include abdominal cramps, nausea or constipation which make life extremely difficult to lead at times preventing moving, travelling, work, events, going out, relationships, friendships.

Travellers’ diarrhoea often but not always due to poor hygiene or contaminated food such as fruits and water; possibly triggered by the change in the climate or a different diet with foods your system is unaccustomed to digesting.

To prevent travellers’ diarrhoea follow these widely known precautions:

  1. Drink bottled, boiled or purified water – even in UK if you  are a chronic sufferer or are pregnant; unless from a known clean drinking source in your, family or friends home
  2. Avoid salad, raw vegetables and unpeeled fruit– especially if pregnant even in UK to protect from Toxoplasmosis is an infection due to the parasite Toxoplasma gondii or this can cause stillbirth
  3. Avoid ice cubes, ice cream and unpasteurised milkListeria
  4. Avoid snacks from roadside vendors – unknown parasites, infections due to lack of hygiene and/or cleanliness
  5. Always make sure food is served piping hot – kills bacteria if over 75 degrees including serving in a hot dish on a carvary
  6. Avoid food exposed to flies or to the sun – even in UK this occurs in shops/stores; buy cheeses, cold meats, delicatessen, fish from Waitrose where covered and closed counter
  7. ALWAYS pack a diarrhoea remedy – I was in the Brownies and Girls Brigade and what I most remember is to ‘BE PREPARED

The main causes which I personally have found to cause severe and mild bouts of diarrhoea are:

  • Iron supplements can cause this in some whilst some may suffer from constipation
  • Steatorrhea is caused by a malabsorption of fats (pale, poorly formed, floating stools)
  • Drugs especially anti-biotics (bio yogurts can aid to rectify this)
  • Irritable Bowel Syndrome – IBS
  • Removal of any part of the small or large intestines
  • Food intolerances, artificial sugars or additives
  • Stress
  • Lack of sleep

Lastly, less common but a must to consider to rule out more so people over 50 are considered more of a risk of Bowel disease.

There are three types of bowel disease which also result in diarrhoea, as a result it is easy to overlook it and treat it as acute diarrhoea. It is therefore of the utmost importance and could save yours or someone else’s life if diarrhoea continues for more than a few days discuss with your doctor to rule these out as another phrase is ‘To be fore-armed is to be fore-warned’

The three types of bowel disease are; Ulcerative Colitis, Crohn’s disease and bowel cancer 

Other symptoms associated with these diseases include; weight loss, blood in your motions and severe abdominal pain. These conditions are rare but serious and you should seek immediate professional medical advice if you think you are suffering from one of them.

However DO NOT PANIC – I have suffered these symptoms without having these conditions but been checked by the doctor.

~ How do you cope with it?~

  1. Seek medical advice if it continues for more than three days due to loss of fluids and nutrients
  2. Research your symptoms for ways to ease it if it is only diarrhoea. I have posted here herbal teas which help as does the BRATBananas, rice/rice water, apples, toast with no dairy.
  3. If it is a food intolerance use an elimination diet

~ Is there an effective treatment? ~

Personally, for me NO! The doctors have been unable to do anything for me, despite many appointments and investigative procedures.

I have found my way to live which works if within my diet and eliminate gluten, dairy, some fruits and vegetables has enabled me to live care for my family fairly easily, without too much discomfort and work.I have found that Alpro Soya Bio Dairy free Yogurt does enable my intestines to be more comfortable as does drinking enough water, camomile  tea and peppermint tea as well as enough rest and less stress!

However it was a hurdle, to find the causes and proceed to cut out anything causing the symptoms and seek to maintain a stable weight and manage my symptoms. There are some symptoms like migraines and monthly effects which I cannot remove due to hormones,which are out of my control – I did see the doctor but they could only delay this and not remove it unless I removed reproductive organs. Although difficult to manage we have to somehow manage to live our life as best we can and consider that there are many people who have little food which helps me keep my ‘troubles’ in perspective.
